Nearly four years after Jeffrey Epstein, the world’s best-connected pedophile, reportedly killed himself in his jail cell, the Justice Department’s watchdog is blaming the guards’ misconduct for the death.

Inspector General Michael Horowitz laid several violations of policy that set the scene for how Epstein managed to take his own life while on suicide watch. For example, a surplus of bed linens were left in his cell, and the convicted criminal was later found hung with one of these linens. Epstein also wasn’t assigned a cellmate, even though he’d made a first suicide attempt nearly two weeks before.

Additionally, nearly all the surveillance cameras on his unit weren’t recording, and he was even allowed an unmonitored phone call the night before his death, which violated policy.

Yet, Horowitz agrees with other investigation findings that there was no foul play in Epstein’s death. Ultimately Horowitz recommended criminal charges against four of the workers, as he says their misconduct “effectively deprived Epstein’s numerous victims of the opportunity to seek justice.”
